Make an Appointment
Patients are seen by appointment – on the same day the appointment is made if possible. Our physician might be in the operating room. If needed, we will schedule an appointment for a "work-in" and will try to have a physician available. To schedule an appointment, please call (305) 933-9440 during regular business hours. When your request is received, our office will contact you by telephone to confirm your appointment time.
Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.
Follow-up Appointments - These are for non-urgent, chronic medical conditions and medication refills. Please call in advance to get the appointment you prefer with your regular healthcare provider. We recommend that you make your next follow-up appointment before you leave the office. Also, try to make arrangements to have any lab tests completed prior to your visit.

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
Patients are responsible for obtaining referral authorizations from their Primary Physicians. Please make sure you have your copy the day of your appointment. Patients without their referrals will need to be rescheduled.
Some insurance companies may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.
We require payment of your deductible, co-payment, and past-due account balances at the time of service. If you do not have insurance, full payment is required at the time of service. We accept cash, checks, debit and all major credit cards. Payment plans are available if arrangements are made in advance with the office.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. We reserve the right to charge $25.00 for "no-show" appointments.
We prefer to refill your medications at the time of your office visit. Please bring your medication list with you to each appointment. Please be sure to give us at least 48 hours notice on all refill requests.
Chronic or long-term medication refills require an office visit before they may be refilled.
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number. Kindly give us at least 48 hours advance notice when requesting copies of medical records.
